Our 116-bed, full-service nursing facility and rehabilitation center offers the best clinical care for residents and patients who require 24-hour attention. The Medicare and Medicaid approved facility offers tastefully furnished private and semi-private rooms in a variety of settings.
Rehabilitation Services - Transition to Home Unit
The transition from hospital to home can be difficult. Our rehab unit and state of the art rehabilitation gym is designed to promote patient return to prior level of function after surgery or serious illiness. Our highly specialized Rehab to Home team provides exceptional short-term physical,occupational,respiratory and speech therapy, nursing care and patient education to help patients recuperate, gain strength and return home as quickly as possible. Other services included in this unit are qualified skilled nursing care, pastoral care, a licensed dietitian and support to promote transition to home for our patients.
Our rehabilitation services are available to all residents of Christian Care Communities campus. We are also available on an outpatient basis to the general community.
Admissions
Our Admissions Coordinator works with all residents and families to ensure a smooth transition into our health center. We start with a thorough assessment of the potential resident’s current medical status and capabilities to determine the appropriate level of service to meet his or her need.
