Boone, Campbell and Kenton counties, Christian Care Communities’ Pathways
Northern Kentucky program is dedicated to offering adults
and children with developmental disabilities community alternatives
to institutional care, so they can live their lives with
dignity, fully engaged in the community around them.
Our Pathways Northern Kentucky program provides living arrangements
and care options tailored to each person’s unique needs.
Options include:
Group Homes & Staffed Residences
Our group homes and staffed residences offer individuals with developmental
disabilities the opportunity to lead fulfilled lives and become contributing
members of their communities.
Adult Foster Care Provider
We match individuals with developmental disabilities with Adult Foster
Care Providers who can care for them on a personal basis, offering care that
changes as the individual’s needs and aspirations change.
Behavior Intervention
When challenges arise, Pathways offers intervention to help both the
individual and caregivers overcome obstacles to community involvement.
Community Living Supports
For those who live independently or with their natural families,
Pathways can provide a variety of in-home training services
to help the program participant with social skills and
daily living skills, in addition to a number of other practical
services.
Community Supports
In contrast to our traditional residential services, our
community supports program allows individuals with developmental
disabilities to live independently in their own residence
or apartment. Trained staff come to help them with
specific needs, such as paying the bills, going to the
grocery store, or going to doctor’s appointments. The
hours of help can vary greatly depending on the individual’s
strengths and needs, and each support program is fully
customized.
Respite Services
Pathways also offers alternative care at planned intervals
or during emergency situations. Our network of Respite
Providers and Adult Foster Care Providers can care for
individuals when another provider or caregiver needs a
break. This allows program participants to maintain
a sense of daily routine, build new relationships and experience
new activities.
Weekend Respite Services Program
Our Weekend Respite program allows individuals with disabilities to truly “get
away” for the weekend at our fully staffed home in Independence, Kentucky.
We offer each individual a wide range of activities, which may include dining
out, a trip to a local festival or park—or staying in to enjoy board
games and rented movies. All weekend activities are matched to individual
preferences. The program, which is open most weekends of the year,
runs Wednesday through Sunday. For reservations or more information,

Case Management
Pathways staff are advocates for individuals with developmental
disabilities, monitoring services, advocating for their
needs, and insuring quality of care.

Would you like to be an Adult Foster Care Provider?
For many adults with developmental disabilities, their
fondest wish is for a stable, loving home environment that
is equipped to meet their special needs. At Christian
Care Communities, it is part of our mission to match individuals
with developmental disabilities with those loving families,
providing the training, support and payment necessary to
make these arrangements possible.
For the individuals supported, they achieve what they
need most—a connection to a home and community, with
the services they need to live as independently as they
can. For the Adult Foster Care Providers, they gain
a new family member/,a friend, and someone whose personal
gifts and affection can add a whole new dimension to their
family lives.
Currently, our Pathways program has more than 370 program
participants with developmental disabilities placed with
Adult Foster Care Providers in Kentucky. Some of
these individuals are quite independent, holding down jobs
in the community or doing volunteer work. Others have more
medically based needs and require more regular care. But
regardless of their abilities, we have found that our Adult
Foster Care Providers have forged meaningful relationships
with the individuals, making them part of their home for
years, if not decades. These individuals often maintain
close relationships with their biological families, as
well, but their Adult Foster Care Providers play a very
important role in their lives.
- How does the home care provider arrangement work?
To
be eligible for a family home provider, an individual with
developmental disabilities must first qualify for a reimbursement
through the Kentucky division for Mental Retardation. Once
eligible, this allows the provider to receive Medicaid,
Social Security or disability payments to pay for the program
participant’s room, board, clothing and personal
care, as well as any additional services they may receive
such as Occupational Therapy or Physical Therapy. Family
home/adult foster care providers receive a regular stipend
for their help.
- Who is qualified to be an Adult Foster
Care Provider?
The state requires that Adult Foster Care Providers be
at least 18 years old with a high school diploma or GED.
- Do Adult Foster
Care Providers receive any special training?
Yes. Pathways conducts specialized training for
people who wish to become Adult Foster Care Providers. The
training prepares potential caregivers for what to expect
from the individual they will support, the details of how
to care for them, as well as any monthly record keeping
or paperwork that needs to be maintained.
In addition, Pathways conducts extensive screening to ensure
that the Adult Foster Care Provider is truly ready to accept
an individual with no prejudices, and treat them as part
of the family. Everyone in the household, including
children over the age of 5, will be interviewed to ensure
they are ready for the changes this new arrangement will
bring. A thorough check of criminal history, a drug
screening and credit check will be performed.
- Are there ever circumstances when an Adult
Foster Care Provider arrangement is terminated?
Yes. Pathways can terminate an Adult Foster Care
Provider for violating regulations they are required to
uphold. The providers receive an annual review, and
are regularly checked by Pathways staff. Also, an Adult
Foster Care Provider can sever their ties by providing
a 30-day written notice to Pathways. However, we
do strive to find foster care providers who are willing
to think of their caregiving as a long-term commitment,
and not a temporary job.
- What if my family needs a break?
Pathways has constructed a network of respite services
that can give foster care providers a break.
- How do I apply to be an Adult Foster
Care Provider?

Who Qualifies/How to Apply
Pathways is a multi-service provider, certified through
the Department of Medicaid Services to offer services to
individuals with mental retardation and other developmental
disabilities. We receive a variety of funding sources for
the supports we provide. All individuals must meet
the qualifications for the corresponding funding source. For
