The transition from hospital to home can be difficult. Our rehab unit and state-of-the-art rehabilitation gym are designed to promote return to independence after surgery or serious illness. Our highly specialized Rehab to Home team provides exceptional short-term physical, occupational, respiratory and speech therapy, nursing care and patient education to help patients recuperate, gain strength and return home as quickly as possible. Other services included in this unit are qualified skilled nursing care, pastoral care, a licensed dietitian and support to promote transition to home for our patients.

Our rehabilitation services are available to all residents of Christian Care Communities.  We are also available on an outpatient basis to the general community.
