Christian Care’s Transitional Care Program helps ease the way home after surgery or illness with exceptional physical, occupational and speech therapies. Our 24/7 clinical nursing care gives peace of mind to patients and their families during this recuperation period.
Patients recovering from major surgery, stroke, joint replacements, pneumonia or other life-changing illnesses or injuries may benefit from our specialized Transitional Care Program. We offer the same in-depth care of a traditional hospital, but for an extended recovery period in a more comfortable environment.
Our goal is to ensure patients are ready to return to life at home upon leaving the Health Center. We encourage active independence while providing trusted support, allowing residents to be a part of a community during their rehabilitation period.
With a Chaplain and attractive Chapel inside each community, providing pastoral support and faith-based care to our residents and staff is a priority.
